The Science

What is Tirzepatide?

Tirzepatide is the first medicine that activates two gut-derived hormone receptors simultaneously — GIP and GLP-1. This dual mechanism produces stronger appetite regulation and metabolic effects than single-pathway GLP-1 medicines like Semaglutide. The molecule is manufactured by Eli Lilly and is marketed under two brand names depending on indication.

GLP-1 Receptor

Slows gastric emptying, suppresses appetite, improves insulin response after meals. The same pathway as Semaglutide (Wegovy, Ozempic), but Tirzepatide engages it alongside a second receptor.

GIP Receptor

Improves fat metabolism and insulin sensitivity. Tirzepatide is the first approved medicine to engage this pathway therapeutically — the addition responsible for its stronger results versus GLP-1-only agents.

Combined Effect

In SURMOUNT-1[1] (the obesity Phase 3 trial), patients on 15 mg weekly lost an average 20.9% of body weight over 72 weeks — substantially more than the 15-17% range typical of Semaglutide[3].

Titration Protocol

Standard Dose-Escalation

Tirzepatide follows the manufacturer titration schedule — incremental dose increases minimize gastrointestinal side effects and allow your body to adapt to the appetite-suppressing effect.

Weeks 1-4
2.5 mg weekly
Adaptation phase — not a therapeutic dose. Allows your GI system to acclimate.
Weeks 5-8
5 mg weekly
First therapeutic dose. Many patients see meaningful results from this point onward.
Weeks 9-12
7.5 mg weekly
Up-titration if tolerated. Dr. Kenika reviews side-effect profile before each step.
Weeks 13-16
10 mg weekly
Intermediate maintenance dose. Many patients hold here long-term.
Weeks 17+
12.5 – 15 mg weekly
Maximum maintenance dose, reserved for patients pursuing the full SURMOUNT-1 efficacy.

Frequently Asked

Common Questions About Tirzepatide

Is this authentic Mounjaro or Zepbound from Eli Lilly?
Yes. Both Mounjaro and Zepbound are manufactured by Eli Lilly and Company. Each pen ships through cold-chain pharmaceutical distribution channels with batch numbers and expiry dates intact. You can verify the manufacturer markings and serial codes directly on the pen before your first injection.
Mounjaro vs Zepbound — what is the difference?
Mounjaro and Zepbound contain the same active molecule — Tirzepatide — manufactured by Eli Lilly. The difference is the regulatory indication: Mounjaro is FDA-approved for Type 2 diabetes (May 2022), and Zepbound is FDA-approved for chronic weight management (November 2023). Pharmacologically they are identical. Dr. Kenika selects between them based on your medical history and dose availability.
What are the side effects of Tirzepatide?
Most common: nausea, decreased appetite, and mild GI discomfort during the first 4-8 weeks of titration. These typically diminish as your body adapts. Slow titration (starting at 2.5 mg weekly) significantly reduces these effects. Rare considerations include pancreatitis history and personal or family history of medullary thyroid carcinoma — reviewed during your consultation with Dr. Kenika.
How do I store Tirzepatide at home in Thailand?
Refrigerator storage at 2-8°C (36-46°F). The pen can be kept at room temperature (below 30°C) for up to 21 days once in use. Do not freeze. Do not expose to direct sunlight. In Thai climate, never leave the pen on a car dashboard or balcony. Our nurse demonstrates correct home storage during your first injection.
How does Thailand pricing compare to the US, UK, and Australia?
Thailand pricing for authentic Tirzepatide is meaningfully below US cash-pay rates and competitive with UK and Australian private-pay markets. Exact figures depend on dose, duration, and shipping requirements — we provide a clear written breakdown during your consultation. We do not publish exact figures online to comply with Thai pharmacy advertising regulations. For international shipping options, see our international patient page.
Can I switch from Ozempic or Wegovy to Tirzepatide?
Yes — this is one of the most common transitions Dr. Kenika manages. Tirzepatide adds GIP-receptor activity to the GLP-1 mechanism you are already on. She reviews your current dose, response, and side-effect profile, then recommends a tailored starting dose. Most patients begin Tirzepatide at 2.5 mg even when transitioning from a maintenance Semaglutide dose. Read more about the comparison on our Semaglutide Thailand page.
What should I do if I miss a dose?
If you remember within 4 days of the missed dose, inject as soon as possible and resume your normal weekly schedule. If more than 4 days have passed, skip the missed dose and take your next dose on the regular day. Never inject two doses within 3 days. Document missed doses to discuss with Dr. Kenika at your next check-in — repeated missed doses may require titration adjustment.
