A century of peptide engineering, distilled into one molecule

Wegovy is semaglutide dosed at 2.4 mg weekly. Behind that single specification sits a Danish pharmaceutical lineage stretching back to 1923 — and a deliberate decade of half-life engineering that turned a 2-hour native peptide into a 165-hour weekly drug.

1923 Nordisk Insulinlaboratorium founded in Copenhagen by August and Marie Krogh after their licence from Banting and Best. The competing Novo Terapeutisk Laboratorium opened two years later. The two merged in 1989 to form Novo Nordisk.
Today World's largest insulin manufacturer by volume. Roughly half of all insulin used by people with diabetes globally is produced inside the company's Danish, French and US fermentation sites.
2005 Liraglutide (Victoza) — the company's first long-acting GLP-1 analogue. A fatty-acid side-chain anchored the peptide to circulating albumin, extending its half-life to about 13 hours and enabling once-daily dosing.
2017 Ozempic FDA approval — semaglutide, an extensively re-engineered second-generation GLP-1 with a half-life of approximately 165 hours. Once-weekly dosing became practical for the first time at this stability.
June 2021 Wegovy FDA approval — the same semaglutide molecule, up-dosed to 2.4 mg weekly, indicated for chronic weight management in adults with obesity or overweight plus a comorbidity. Based on the STEP-1 phase-3 program.
Mar 2024 Cardiovascular indication added — Wegovy becomes the first weight-management drug ever to carry a FDA-approved cardiovascular-risk-reduction label, on the strength of the SELECT outcomes trial. See the next section.

If you have established cardiovascular disease and are not diabetic, Wegovy is the only GLP-1 agonist on the market whose label explicitly includes reducing the risk of a heart attack, stroke or cardiovascular death. That distinction comes from the SELECT trial.

Primary endpoint

Lincoff and colleagues (NEJM 2023) enrolled adults aged 45 or older with body-mass index of 27 or higher, established cardiovascular disease, and without type-2 diabetes. Patients received either semaglutide 2.4 mg weekly or matched placebo on top of guideline-recommended standard care.

The primary composite endpoint — non-fatal myocardial infarction, non-fatal stroke or cardiovascular death — occurred in 6.5% of the semaglutide arm versus 8.0% of the placebo arm. Hazard ratio 0.80, 95% confidence interval 0.72 to 0.90.

In March 2024 the US FDA expanded the Wegovy label to include this indication. As of mid-2026 no other GLP-1 agonist — not Mounjaro, not Ozempic, not Saxenda — carries an equivalent regulatory statement for cardiovascular-event reduction in the non-diabetic population.

Wegovy reaches its 2.4 mg target dose in 17 weeks — three weeks faster than Mounjaro's 20-week schedule. The titration uses doubling and roughly-doubling jumps every four weeks, with a 1.7 mg intermediate step that flattens the climb to the maximum dose.

If 2.4 mg is not tolerated, the prescribing doctor may hold maintenance at 1.7 mg long-term rather than de-escalating to 1.0 mg.

Who Wegovy fits

Strong fits and weaker fits, stated plainly

Wegovy is not the strongest weight-loss number on a clinical-trial chart — that is Mounjaro. But it carries the longest safety dataset in the GLP-1 weight-management class and the only cardiovascular outcomes label. Decide accordingly.

STRONG FIT

Wegovy is the right starting molecule for

+Patients with overweight/obesity plus cardiovascular risk factors — established coronary disease, prior MI, prior stroke. Wegovy's CV indication is genuinely unique in this class.
+Patients without type-2 diabetes who fit the STEP-1 phenotype (BMI 30+, or 27+ with comorbidity). This is precisely the cohort in which the 14.9% weight-loss number was generated.
+Patients prioritising the longest post-launch real-world safety dataset in the weight-management GLP-1 class — Wegovy has been in continuous clinical use since 2021.

WEAKER FIT

Consider a different GLP-1 if you are

−Targeting maximum weight loss (more than 20%). SURMOUNT-1 tirzepatide 15 mg outperforms STEP-1 semaglutide 2.4 mg in head-to-head benchmarks. Mounjaro is the stronger molecule by this metric.
−Already managing type-2 diabetes. The same semaglutide molecule is sold as Ozempic with a T2D label and broader supply availability — start there if diabetes is the primary indication.
−Severely cost-constrained. Saxenda's per-shot rate in Thailand is roughly one-third of Wegovy's per-week equivalent. The trade-off is daily injections and a smaller efficacy ceiling.

Wegovy is the Obesity-Indication Brand

Wegovy is FDA-approved for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27) plus weight-related comorbidities such as hypertension, dyslipidemia, or sleep apnea. The molecule — Semaglutide — is the same one marketed under the Ozempic brand for Type 2 diabetes, but Wegovy uses a higher dose range (up to 2.4 mg vs Ozempic's 2 mg).

Common Questions About Wegovy

What is Wegovy HD (semaglutide 7.2 mg)?

Wegovy HD is a higher-dose semaglutide (7.2 mg once weekly) that the US FDA approved in 2025 for adults with obesity who have already tolerated the 2.4 mg dose for at least four weeks and still need further weight reduction. In the STEP UP trial it produced about 20.7% mean weight loss over 72 weeks (versus roughly 14.9% at 2.4 mg), with a higher rate of altered-skin-sensation side effects. It is a separately approved dose — not something to improvise by taking more of the standard pen. As of mid-2026 Wegovy HD is not registered in Thailand; whether a higher dose is right for you is a decision made with your doctor.

Is there a Wegovy pill (oral semaglutide)?

Yes. In December 2025 the US FDA approved the Wegovy pill — once-daily oral semaglutide 25 mg — the first oral GLP-1 for weight management. In the OASIS 4 trial it produced about 16.6% mean weight loss, similar to the injectable 2.4 mg pen. It is rolling out first in the US and has been submitted to other regulators. As of mid-2026 the oral form is not yet available in Thailand — our program uses the authentic FlexTouch injectable pen, and we will update patients when the pill reaches Thailand.

Does Wegovy protect the heart?

Yes. Wegovy is the first GLP-1 with an FDA-approved label to reduce the risk of major cardiovascular events (heart attack, stroke) in adults with obesity and established cardiovascular disease, based on the SELECT trial. This cardiovascular indication is unique to semaglutide among the weight-loss GLP-1s.

How does the Wegovy dose schedule work?

Wegovy is a once-weekly injection, titrated gradually over about 16–20 weeks: 0.25 → 0.5 → 1.0 → 1.7 → 2.4 mg. The slow ladder lets your body adapt and reduces nausea; your doctor sets the pace to your tolerance.

Is Wegovy available and legal in Thailand?

Yes — at TRT Bangkok, Wegovy is prescribed and supervised by our doctor, with cold-chain delivery nationwide. It is dispensed under medical supervision, not by mail-order or self-titration.

What should I expect in the first few weeks on Wegovy?

The most common effects are mild, dose-related nausea, fullness or digestive changes, which usually ease as your body adapts. The gradual titration is designed to minimise them, and your doctor reviews each step before increasing your dose.
