Two paths to clinical eligibility

FDA and EMA guidance for GLP-1 weight-management therapy uses two BMI thresholds. Most adults seeking treatment qualify under one of them.

BMI ≥27

Overweight range, with at least one weight-related condition (see conditions grid below).

BMI ≥30

Obesity class I or higher, no additional conditions required for therapy consideration.

BMI is a starting filter, not the full picture. Dr. Kenika reviews waist circumference, body composition, and metabolic markers before prescribing.

Weight-related conditions that qualify at BMI ≥27

If your BMI is between 27 and 30, you still qualify for GLP-1 therapy when any one of the following co-morbidities is present.

Type 2 diabetes

Confirmed T2D diagnosis, HbA1c ≥6.5% or on glucose-lowering therapy.

Prediabetes

HbA1c 5.7–6.4% or fasting glucose 100–125 mg/dL.

Hypertension

Sustained BP ≥130/80 mmHg or on antihypertensive medication.

Dyslipidemia

Elevated LDL, low HDL, or triglycerides ≥150 mg/dL.

Obstructive sleep apnea

Diagnosed OSA, AHI ≥5, with or without CPAP therapy.

NAFLD / MASLD

Non-alcoholic / metabolic-associated fatty liver disease on imaging or biopsy.

PCOS

Polycystic ovary syndrome with insulin resistance or weight component.

Who should not start GLP-1 therapy

These contraindications are reviewed during the doctor consultation. Absolute contraindications rule out GLP-1 therapy entirely; relative contraindications require careful case-by-case judgement.

Am I eligible for GLP-1 weight-loss therapy?

You're eligible if you have BMI ≥ 30 (obese), or BMI ≥ 27 with at least one weight-related condition (hypertension, type 2 diabetes, dyslipidemia, OSA, established cardiovascular disease). Adults 18+ only. Dr. Kenika confirms during eligibility consult.

What conditions exclude me from GLP-1 therapy?

Personal or family history of medullary thyroid carcinoma (MTC), Multiple Endocrine Neoplasia syndrome type 2 (MEN-2), severe gastroparesis, history of pancreatitis, or pregnancy. These are absolute contraindications per FDA boxed warning.

Do I need a blood test before starting?

Most patients don't. Your eligibility consult with Dr. Kenika determines if labs are needed. Patients with diabetes, kidney issues, or known metabolic conditions need baseline labs (HbA1c, kidney function, liver enzymes). Otherwise — prescription same-day.

What if my BMI is below 27?

GLP-1 medications are not indicated for cosmetic weight loss in adults with BMI < 27. If your BMI is in this range, we'll discuss alternative options including nutrition guidance, body-composition tracking, and metabolic optimization.

Can I take GLP-1 with my other medications?

Most other medications are compatible. Dr. Kenika reviews your full medication list during consultation. Caution with insulin and sulfonylureas (hypoglycemia risk) and warfarin (dose may need adjustment).

What if I had pancreatitis years ago?

Pancreatitis history is a contraindication per the FDA labels. Even decades ago, the risk of re-occurrence with GLP-1 is elevated. We may explore alternative weight-loss approaches in consultation.

Do you treat patients outside Thailand?

Yes — international patients across Europe, UAE, Singapore, Australia. You do labs locally, video-consult with Dr. Kenika, and receive medication via DHL cold-chain. Some countries restrict GLP-1 import — we confirm before shipping.

What happens if I'm not eligible?

We tell you upfront and refund any medication payment. The eligibility consult fee (฿1,500–2,000) is non-refundable as it covers the doctor's clinical assessment time. We'll suggest evidence-based alternatives where appropriate.
